Hello and Welcome
There are 2 questionnaires in total, for you to complete. In preparation for our first session, you will complete the DASS, and then another assessment form that covers your symptoms and background history. These forms allow me to get much of your information before the session, so your session time is fully productive.
You will also receive email boosters plus attached links to videos and audios, handouts of information on Mindfulness Meditation, your Subconscious mind, thoughts and feelings, and other things. These emails are great for helping to keep you on track, and remind you to do what you need to do because there is often homework to complete.Ater yo
Homework may be in the form of mental exercises, practising with your audio relaxation exercises, or just taking better care of yourself and will assist you to de-stress, relax and enjoy Living Life 2 The Max!
After your first appointment, you will also receive the link to several MP3 recordings, such as: 
- Deep Breathing Explained
- Mindfulness Meditation Explained
- Mindfulness Exercise to guide you until you learn it by heart
- Relax Now
- Stay Slim Now
- Clean Air Breather to stop smoking
- Sleep Like a Baby Tonight
There are also short videos on a variety of subjects, most are available right now on the blog posts and for relaxation training.
CANCELLATIONS OR RESCHEDULING
It is essential that you call or text to provide 48 hours notice prior to cancellation or rescheduling of your appointment and will be appreciated.

The time taken depends on how much detail you give. Each attending person (couples) needs to complete all forms.
Remember to save the address of the page you are working on, and copy and past your info into a word document if you like to write, so that if you get interrupted you can find the specific page again, and can copy and paste from your word document into the form, as the form won’t save your information.
It is important you provide sufficient information to give me a thorough understanding of your situation, and to look ahead at your goals. Plus, it will save time in your first session, so we can do some things together to help you feel better straight away. If you like, think of it this way, that it is your way of making a commitment to work with yourself and with me to the fullest extent to help you to be well.
