About the DASS21
The DASS21 is a quick and easy survey of your current depression, anxiety and stress levels. It is not a stand-alone measure of symptoms for a formal diagnosis however it is a reliable and valid measure.
Please complete the following questionnaire by rating the severity of your symptoms for the past 7 days.
Instructions:
Please read each statement and check the number 0 zero, 1, 2, or 3 to indicate how much the statement applied to you over the past week, i.e. the past 7 days. You are examining yourself and providing an average for the past 7 days.
- There are no right or wrong answers.
- Do not spend too much time on any statement. In other words, do not overthink your answers.
- Complete all statements.
- Your responses will be emailed to your psychologist and scored, so please complete this at least 1 day prior to your appointment if you have one scheduled.
- Each answer has been preset to “3”, so please CHANGE the answer to your most accurate response by clicking on the button.
The rating scale is as follows:
0 – Did not apply to me at all
1 – Applied to me to some degree, or some of the time
2 – Applied to me to a considerable degree, or a good part of the time
3 – Applied to me very much, or most of the timeThe DASS21 is the shortened form of the DASS42 but still has good reliability and validity. This questionnaire does not diagnose a mental health disorder. Your results will be provided at a later date so please use a valid email address.
